We do our best to accommodate patients' schedules. Upon arrival, you'll be asked to fill out forms regarding your current medical condition and previous medical history. This is a necessary part of the evaluation and examination process and enables us to complete insurance claims on your behalf.

To save time, you can access the forms online, and fill them out before you arrive. If you do not bring completed forms with you, please plan to arrive 20 minutes before your appointment time to fill them out in the office.
